When an entity receiving a grant requires funding under subsection 12(4.1) or (5) of this Schedule, the grantee shall provide the entity with the following information
(a) The name and address of the entity receiving the grant, the purpose of the grant and a number of items that the grantee may specify. (b) The number of grants received in connection with the program. (c) The name and address of the recipient of the grant, the recipient’s income and the status of each recipient as described in section 111. (d) The name, address, and telephone number of the entity receiving the grant. (e) The name and address of the recipient as described in section 111. (f) The total amount paid to the State on applications for and grants through the program under such subsection (1).
(g) The information described in paragraphs (5) and (6) of subsection 12 (4.1) is available only to entities that are designated by the State to receive the program through the grantee, who, in a process of determination by the State, determines that additional, related financial needs of the recipient are needed. For more information, see (5) of subsection 12(4).

(a) A description of the recipient’s financial records to be used in determining the criteria for approving grants under section 11.1. (b) Any other required documentation that the State considers necessary.
(9) If the State determines that there is insufficient evidence to support an action or prosecution and the applicant files an application for a grant under section 12.2, the State shall make its decision within 4 months after receipt of application by the Federal government. (10) The State shall provide the applicant with the information described in subsections 12(5). (11) If a grantee requests and receives additional information described in paragraphs (5) and (6) of subsection 12 (4.1), the State shall make the available to the State information relevant to that request.
